Little Teeth, Big Decisions: Mastering Pulp Therapy in the Primary Dentition

This lecture covers pulpotomy in primary teeth across three core pillars. Diagnosis comes first and is non-negotiable — differentiating the pulp states using both radiographic and clinical findings determines whether a pulpotomy is even appropriate, since the best technique in the world won’t save a tooth that was misdiagnosed. Technique involves complete coronal pulp removal, hemostasis assessment, and application of a pulpotomy agent — historically was formocresol, but now may be ferric sulfate or biocompatible materials like MTA or Biodentine. The lecture closes by stressing that the final restoration, ideally a stainless steel crown, is just as critical as everything that came before it — without a proper coronal seal, the pulpotomy fails regardless of how well it was executed.

  • Accurately diagnose pulp status in primary teeth using clinical and radiographic criteria to determine appropriate treatment selection.
  •  Execute the pulpotomy procedure using current evidence-based materials and techniques with proper hemorrhage control and pulp space management.
  • Understanding the clinical and radiographic indicators of pulpotomy success and failure to guide follow-up decisions and treatments.
